Before sending the Application Form, please ensure the following are included, failing to do so may result in not processing the Application forward:

• Copy of Birth/Adoption Certificate or Passport
• A Passport size photo
• Recent School/College Report 

Oversea students must also provide:

• Reference from a Professional Person, i.e Doctor, Legal personal, Teacher, Government Minister, Religious Minister etc
• Photo of Parents or Legal Guardian
